SIBO Research Insights

Is thyroid disease a risk factor for small intestinal bacterial overgrowth (SIBO)? A prospective study demonstrates that patients with hypothyroid disease are more likely
than controls to have SIBO, but eradication of SIBO does not affect thyroid hormone levels.

Is gallstone disease associated with small intestinal bacterial overgrowth (SIBO)? A prospective study demonstrates that patients with gallstone disease are more likely than controls to have SIBO and prolonged orocecal transit time (OCTT).

PREVIOUS SIBO NEWSLETTERS - 2015

The importance of testing-- and retesting
by Dr Nirala Jacobi, BHSc, ND (USA)
CMO, SIBOTest

Like you, I see many clients who complain of digestive symptoms.  Besides stool tests for large bowel conditions and  blood spots for food sensitivities, I frequently recommend testing for SIBO with a lactulose breath test.
In most cases, these clients have already tried several protocols or have been to other practitioners who have not done functional diagnostic tests and are interested to find  the cause of their symptoms once and for all.

I always explain the benefit of testing rather than just treating for SIBO.

1. The breath test helps us identify if the problem is in the small intestine vs the  large intestine.
2. The test helps us to know if the patient produces Methane gas or Hydrogen gas or both.  This is important so the practitioner can select the right antimicrobial.
3.  The test helps us determine how high these gasses are so we can have a relative accurate prediction of how long the treatment might be. Prediction of length of treatment tends to be quite accurate with Methane elevation, and less accurate for elevated Hydrogen.  Each course of recommended Herbal antimicrobials (as described in the treatment protocol) reduces methane by 30-50ppm.  If you have a patient whose methane levels reached 100ppm you will need to consider prescribing 3 rounds of a 6 week herbal protocol to drop methane to acceptable levels.

Retesting

Because relapse of SIBO is so common, retesting is important after the patient has completed the 6 week herbal antimicrobial protocol.
This is recommended by SIBO experts like Dr Mark Pimentel (gastroenterologist, SIBO researcher) and Dr Allison Siebecker, ND of the SIBO clinic in Portland USA.

1. Retesting ensures the complete eradication of bacteria so the patient can commence the maintenance program.
2. If eradication has not occurred - the practitioner needs to know how much the gas levels have dropped to assess effectiveness of herbal antimicrobial.
3. Once bacteria are truly eradicated, you can place patents on the maintenance diet and prokinetic products.

This approach is an almost fool-proof recipe for successful SIBO treatment.

NEW RESEARCH ON SIBO

In this first of its kind study through Johns Hopkins Hospital, researchers found herbal antimicrobials to be on par with Rifaximin (Xifaxan) for the treatment of SIBO. The authors concluded that  " Herbal therapies are at least as effective as rifaximin for resolution of SIBO by LBT (lactulose breath test). Herbals also appear to be as effective as triple antibiotic therapy for SIBO rescue therapy for rifaximin non-responders".

Herbal therapy is equivalent to rifaximin for the treatment of small intestinal bacterial overgrowth.
